Fanapt® dosing and titration

A dose range of 6 to 12 mg twice daily was efficacious for most patients1
  • The maximum recommended dose is 12 mg twice daily (24 mg/day) taken without regard to meals
Fanapt® titration may help patients achieve an efficacious dose in as few as 4 days1

The recommended dosage for CYP2D6 poor metabolizers is 3 mg to 6 mg.

 

Titration pack for therapy initiation

Fanapt® must be titrated from a low starting dose to avoid orthostatic hypotension1
  • Slow titration of Fanapt® (iloperidone) may reduce dizziness, tachycardia, and syncope, which are associated with orthostatic hypotension

 

Consider metabolic pathways

In certain circumstances, Fanapt® dose should be reduced by one-half1
  • When patients are CYP2D6 poor metabolizers
  • When administered concomitantly with strong CYP2D6 or CYP3A4 inhibitors
    • When the CYP2D6 or CYP3A4 inhibitors are withdrawn from the combination therapy, Fanapt® dose should be increased to where it was before
QT prolongation was augmented by the presence of CYP2D6 or CYP3A4 inhibition1
  • The use of Fanapt® should be avoided in combination with drugs known to prolong QTc and in patients with histories of significant cardiovascular illness
